A 42-year-old man who is HIV-positive is being treated for his first episode of Pneumocystis carinii pneumonia on an inpatient medical ward when he begins to become increasingly disoriented, combative, and agitated. Pulse oximetry shows that he is oxygenating at 93% on room air. He is currently receiving nortriptyline for depression and gabapentin for pain due to peripheral neuropathy. In a patient such as this with a medical illness, a history of the acute onset of agitation, disorientation, and confusion is consistent with and most suggestive of the diagnosis of delirium.

IF you think about it, patient with alcohol withdrawl goes thru, sweating , palpitations, in short tremens.. right.. and this guy seemed more so disoriented, combative etc...so by comparing the two I would pick E

I still feel its alcohol withdrawl.You see all the stuffs mentioned above in alcohol withdrawl.

Multifactorial delirium from what???Whats causing the delirium??
