An african woman go to your office with diagnosis of sarcoidosis. You need to prescribe an diuretic, Which one is most appropriate ?

A. Thiazide

B. Acetazolamide

C. Manitol

D. Furosemide

i don't get the clue from sarcoidosis.....so ill go with the first line of agent

I will go with D ) Furosemide.

in sarcoidosis there will be hypercalcimia and thiazides increase calcium levels .

Of course, kmp has it right and is giving good information "Lovely Lasix"!

keys:
1)ACE levels
2)hilar adenopathy
3)E. nodosum in a relatively young black female
4)noncaseating granuloma

Also Acetozolamide would tend to put her toward metabolic acidosis
Mannitol effectively dilutes intraluminal sodium conc. and markedly
INCREASES tubular flow rate. Extra-*mannitol and urea are osmotic agents
known for being 2 of the most rapid and freely filtered agents at the glomeruls!

And sarcoid macrophage secrete 1,25 Vit D :arrow: Hypercalcemia
