Isoflurophate, Echothiophate, Parathion-(Organophosphates)
Edrophonium, Physostigmine, Neostigmine

Mech-inhibits acetylcholinesterase-neuronal response to Ach is increased

Only the organophosphates irreversibly inhibit acetylcholinesterase.

Organophosphates were used in the war as nerve gas.They produce immense stimulation at cholinergic receptors producing respiratory paralysis and CONVULSIONS.

Isoflurophate and echothiophate are used occasionally for glaucoma
esotropia.
Toxicity-Excess cholinergic stimulation.

Physostigmine used for glaucoma,2nd choice after pilocarpine and for
overdosages of atropine, phenothiazines, tricyclics
intestional atony, and rarely for esotropia. Crosses BBB
can ccause cataracts and CONVULSIONS.

Neostigmine does not cross BBB, Used for Rx of M.gravis ,urinary ret. and
paralytic ileus, the antidote for tubocurarine- pyridostigmine
is similar with duration of action of up to 6hrs.

Edrophonium Used to DX M.gravis and to differentiate Myasthenia gravis
from cholinergic crisis.

Donepezil and tacrine are newer agents that cross BBB and Rx Alzheimer

Organophosphates -malathion and parathion are insecticides,sarin is the nerve gas, and echothiophate is used for glaucoma and accommodative esotropia
