50yrs female +fever+ greenish spututm+ no other comorbidities+ cxr consolidation r lower lobe..... PR 80 , RRATE 18, BP NORMAL TEMPRATURE 101 if we say community acquirred pnumonia whats the rx :?: kaplan says... erythro. uw says vanco+ceftriaxone+ampicillin ... AND I SAY HELLLLP ME

To me ampicillin seems to be used first in most cases, you know in community acquired uncomplicated simple pnuemonias in previously healthy individuals assuming that it's a typical pneumonia. But if it's an atypical then erythro would be the choice......i think. But this all for in outpatient practice. To use big guns like vanco and cephalos the patient has to be pretty in bad shape and in a hospital...... but who knows. I am all friking confused with these stupid stuffs, did i tell you i hate these tests? I think i did, but anyway again i do hate these tests. God help us! [-o<

I do hate them too ELM more than you I think :x . But we have no way out ! We have to do them anyway... But listen "we have to make our goal the eternal quest to perfect the Art of Medicine in service of Humanity" If we can remember that someday we can relieve the suffering of one individual or save the life of people from what we learn then we should be HAPPY :lol: MLE are just road bumbs of which we have to pass thru them anyway... Isn't greenish sputum specific for Haemophilus infection :roll: ? If yes, Erythro is not effective in most cases. Need more powerful ABs.

Hi, Noelle, Thanks for bring up the q, I am not on Pul yet, but this is really an Important issue: a common lung infection! I did some research, found that there is an American Infectious Dis Guidline for "community acquired pneumonia", Lengthy! If I remember right, you are going to take the exam soon, so just introduce some specific points to this q, for more, pls read the guidline. The list of pathagens for this condition is more than one page! To treat out-pt with unknow pathogens (in this case), macrolides is the first chioce next is fluroquinolones or doxycycline. Vancomycin will be used only for in-pt (step1 Kap Pham) and for pathogen-specific Tx (guidline). I am not sure what kind of cs you had in UW. Hope this help!
