don't worry about it , I am IMG too and took this silly test a few month ago sometimes I couldn't express myself during the test and found proper word at that moment but I just tried to act like an actor you should think about it that you are visiting this patient in your country and you are going to get some information from him, sometimes body language is the key but anyway I passed this test and I tell you it is nothing just be yourself and don't worry about english that much

donīt worry about it, Iīm in the same boat and I passed the step 2 CS . I donīt fully understand moviesī dialogues and TV shows. Itīs considered that if you are able to understand and pass the USMLE step 1, you have the minimum required level of english to pass the step 2 CS, according to the Kaplan guide for the CSA.
Just practice a lot of simulated cases (check the cases from the First Aid for Step 2 CS).

Yes, It does not really matter if you don't understand TV shows completely. Our medical language is totally different anyways and people of the non-medical field don;t understand us either.
Something that is extremely important is how you convey the information. Yes, body language is very important. You have to smile and make the patient feel confortable, have eye contact at the time you are asking them questions and always "smile" at them. IN this test they are not going to evaluate your grammar, so keep it simple...simple, phrases, straight questions, one by one, don;t ask 2 questions at the same time, you also try to avoid using medical words because the patient will not understand what you are talking about. Keep it simple, ask one question at a time and show your "teeth". grin
