Botulism shows incremental increase in muscular fiber contraction (opposite of Myasthenia Gravis). So the answer is B, Eaton-Lambert Syndrome.

Both Botulism toxin and Eaton-Lambert auto-antibody bind to presynaptic membrane and prevent the release of Ach. MG anto-antibody attacks postsynaptic membrane.

Hi ELM, Thank you for remembering me. I am not ready to take step 2 yet, but I aim at May or June. I think I was a little overjoyed with step 1 result last year and spent too much time to celebrate. Anyway, now I am forcing myself to concentrate on study. How about you?

Hi, ELM, thanks for introducing latitude610 to us. I missed his step1 posts, but I just went back and read all his posts. I was shocked!

TO Latitude610, welcome back to step2. We are very happy to have you here. I think you need to update your UN to latitude730! Your score is an incredible NBME history! I am sure you will make a new record for step2. I am working to death trying to sit around June or July (just started a mo). It would be great if we could share with your strategy and your new posts in the coming mons. In particular, myself is an 80s' graduate, have two kids, so your experience is very helpful for me. Will talk to you later.
