50 y/o male comes in complaining of chronic back pain and feeling tired. He also has had a history of history of infection with Peumococcus and Haemophilus. Previous protein elctrophoresis results showed he has a marked monoclonal IgA spike. What is the most likely diagnosis:

a) waldstrom
b) multiple myeloma
c) CML
d) metastatic ca

B, mult/myeloma.

Lucky you got to have monoclonal spike of IgM for Waldenström. (notice the M upside down)
This is multiple myloma.

Yes. In multiple myeloma, there can be:
1) IgG spike
2) IgA spike
3) or combination

Plus in MM encapsulated bac infections are common. 8)
