Since his last immunization record is > 10 years, I assume you have to give him Tetanus toxoid

if pt has received<3 doses of tetanus toxoid or imm hx is unknown
Td fr clean minor wounds
Td and TIG fr all other wounds

if pt has received 3 or more TT doses,

clean wound -Td(if last dose >10 yrs ago)
all other wounds -Td(if last dose >5 yrs ago)
