Secretory diarrhea - increased secretion of fluid and electrolytes ( may be caused by activation of adenyl cyclase-cAMP system ) or decreased absorption . Adenyl cyclase-cAMP system is activated by cholera toxin , E. coli toxin , Salmonella enterotoxin and VIP . Osmotic diarrhea - caused by nonabsorbable substance in the lumen of intestine with secondary accumulation of fluid .

and -osmotic diarrhea stops on fasting but secretory does not -osmotic gap is >50 in osmotic diarrhea and nl in secretory. [osmotic gap is measured osmolality - calculated osmolality] calculated osmolality-- 2(Na + K)
