The timings are important because the ECFMG requires that you have spent so much time in a given posting, so many months in medicine, in surg, psych, etc. I don't think they actually double check that info. I had the original schedule issued to me by my college, so it wasn't hard for me. As for dept heads, I found that on the internet.
I would say, just make sure your timings give you enough time in each subject as the ECFMG requires. I don't think dept heads matter than much. Just write your Dean's name.
