I think its like an externship. You rotate in a particular hospital service but dont get paid. Even these are not easy to get.

Externship - you do not get paid but you do all the tings residents do .
O bservership - you do not get paid but you just follow a physician and observe what he/she is doing . You do not examine patient yourself .
