Do we check the ankle reflex by tapping on Ahiles tendon, right? I know it is a weired Q, but I think I ve read too much by now, I am getting more confused, then clear... And for orthostatic VS, if I have to check, should I ask patient to lye down, check the pulse and then to sit or stand up and check pulse again? thanks...

You are right about the ankle reflex . Generally you will not have enough time to check orthostatic signs in the exam - you can tell the Pt. that you need to check them and that you are going to do that later . Do not forget to write it down on your evaluation list ! When you do check the orthostatic signs ask the Pt. to lie down , check his BP and PR . Let him lie down for a couple minutes then ask to stand up and check BP , PR again .

if we can write "check orthostatic BP" in the work-up plan, does it mean we will never need to do BP check in the real exam? thanks.

http://www.prep4usmle.com/community/topic13524.ht... it got the answer here. it means we almost don't ever need to do BP check in exam unless we are asked to do it.
