never to be used before initial assessment. that would be something like prescribing a painkiller to a patient with acute appendicitis and sending him home on that! :lol:

since it will interfere with glasgow coma scoring and assessment of sensorium, it is to be used only in special circumstances like -

if the patient is intubated but struggling against the ventilator or thrashing about a bit, one may consider using short acting sedatives like Midazolam, as an infusion, 2mcg/kg/min.
