35 yr old lady present w/ complaints of burning urination for the past 2 days and deep stick test of her urine demonstrate marked positivity for the but no reactivity for the nitrite .urine culture later grows out large no. of organisms. which of the following bacteria is most likely to be responsible for patients infection?

1. enterobacter sp.

2. enterccocus fecalis

3. e.coli

4 klebsiella pnemoniae

5. pseudomonas aeruginosa

plz explain....i don't get this q at all

positivity for wht & wht nitrite test is there for e.faecalis???

When the urinalysis is done on a specimen that has E. faecalis, the dipstick doesn't show a positive nitrate like it does with gram negative rods. Therefore, you cannot assume that a urine with a negative nitrate test is negative for pathogenic bacteria. All the rest of the organisms listed are gram negative rods.

I'm not sure if Pseudo. aeruginosa always produces a positive nitrate on a dipstick (it's been a long time since I've done urinalysis - next time I get a urine with Pseudo I'll check the urinalysis to see) but a 35 year old woman would more likely have E. faecalis in her urine than Ps. aeruginosa. This is just an impression from reading out thousands of urine cultures over the years - I don't have any statistics to back me up on this :| .
