An infant delivered to an 18 y.o heroin user is determined at birth to have an HIV ELISA titer of 1/264 and 3 positive reaction bands by western blot.
Which of the following is the correct interpretation ?

A. The child has been exposed to HIV
B. The child has been infected to HIV
C. The mother has been exposed to HIV
D. The mother has been infected to HIV

Can somebody explain why is D wrong? The Wesern blot is positive.... so the source of antibodies most likely has to be the mother, unless the fetus sero converted in-utero (then E would be correct) Anyone????

Not sure.... but the way I understand it is in the 264 squares microtitre plate even if 1 is positive (ie we have an IgG against p24), we have a positive ELISA screen ie, 1/264 ...then we have to repeat the test and if it comes back positive again we have to do the confirmatory test Western Blot. In WB, if we have a positive band for atleast 2 of the antigens, we can call it positive. Anybody else???

D is for sure the right possible explanation

C is wrong bc once exposed u get an infection

A, B & E are wrong, the test is positive in the infant only bc of maternal igG that crossed the placenta(from an infected mother), the only way to detect HIV for sure in a newborn is to do a PCR

This is a Q bank Question, and the answer is D.

The positive ELISA and Western Blot results indicate that the mother is infected with HIV and IgG antibodies have been transferred from her bloodstream across the placenta to the child. All children born to HIV positve mothers are born with positive ELISA and Western Blot results. Only 20% of children born of infected mothers are actually infected with the virus,however, and it is not possible to diagnose an inutero infection using these tests. There is NO current diagnostic to evaluate "EXPOSURE" of an individual unless one considers the testing of his/her sexual contacts for the presence of infection.

I've never performed HIV testing, and don't really know that much about the serology side, but these two articles look interesting.

well reet, remember that the igg abs of the mother may pass into the child, so u really cant determine from this q whetehr he is exposed or infected, so dont mark that choice
