i would like to add the list:

BP....Blood pressure

P....pulse

R....respiatory rate

T.....temperature

DTR...deep tendon reflexes

PERLA...pupils equal react to light & accomodation

EOM...extraocular muscles

WNL....within normal limits

NKA...No known allergies

NKDA....no known drug allergies

Few more are : WNL, JVD, BS, RRR, PPD, HPI, PMH, OTC, COPD, ACE, CXR etc

i hate to be a stickler here, but when i was doing my neurology rotation, i wrote down PERLA. The Neurologist saw this and became angry with me. He said its PERRLA, not perla, pupils equal ROUND and reactive to light and accomodation.
