Plz explain its hemodynamic effect

  #2

Squatting traps deoxygenated venous blood in the legs preventing it from returning to the heart, raises aortic pressure by obstructing the femoral arteries and so a decrease in the right to left shunt.

rt. to left shunt????????????????

In Tet, there are 4 defects. 1. Pulmonary stenosis, not letting blood go out of right ventricle.

2.VSD, As due to pul stenosis the right vent. pressure is very high compared to left, blood enters left ventricle (Rt to Left shunt), mixing non oxygenated blood with oxygenated(Cyanosis)

3. Overriding of aorta,i.e aorta exits the heart from a position between the right and left ventricles(again mixing of both sides blood leading to cyanosis)

4. Right ventricular hypertrophy, due to pul stenosis and constant back pressure on the right vent.( contributing more to the R to L shunt)

Hope this helps.

sorry buddy ... but i was asking wht has this shunt to do with squatting position ....... as u mentioned

Sq decrease the venous return to rt heart ( helping reduse the presure of blood entring from rt to left side due to low pressure) and by obstructing femoral artery, raise the aortic pressure( so increasing the resistance to blood movement from rt to left, again decreasing the R to left shunt)

This is the best I can explain, may be someone else can explain it better.

oh never mind....my mistake, i was confusing this shunting effect with congenital anomalies....u got tht rite

Hi knasim

I agree with you that squatting cause increase systemic pressure  reduce right to left shunt

But it also cause increase venous return too, not reduce it

http://www.gpnotebook.co.uk/cache/-2026569681.htm ( i check the same idea in Harison too.

In my opinion, i think that when we are in squatting position  Muscle contraction  Increase venous return, i am not very certain

Check this out too and then we can talk about it.

God...two different things on the sme website!

on one hand they say VM inc. the venous return....
one the other hand they say VM traps the deoxygenated blood as benificially evidenced in children with TOF...

very confusing.....
