You're working in the family practice clinic, and a 22-year-old breast-feeding woman presents with a painful mass in the inferior portion of her left breast. Your examination reveals a fluctuant mass that is tender and warm. You can expect green-tinted breast milk on exam. You tell her which of the following?

A. Continue frequent breast feeding and take an antibiotic.

B. Stop nursing and let the infection heal.

C. We will call the surgeon and see if an excisional biopsy can be performed today; we will also set you up with an oncology appointment.

D. We will set you up for a mammogram today.

E. We will set you up with a surgeon who will take a small-needle biopsy just to make sure no cancer is present.

Answer should be B. as this is abcess. During lactation mastitis is a common problem. If there is no abcess formation along with mastitis, breast feeding should be continued. But when the abcess is formed , discontinue breast feeding and give antibiotics. Drainage of the abcess is usually required.

Fluctuant Mass ...

I guess thats not suggestive of a cancer/tumour ...

So it has to be an abcess ... and the answer would be

B) Stop feeding and start anti biotic !

And I guess the Author should write a final answer to the question he/she has posted ! ... makes ppl go ... :evil: ..... :wink: !!
