35 yo female comes to see her physcian after she palpated a lump in her rt breast.She has no other complaints. She hasnt seen a doctor fr past 10 yrs. She states that she regularly self examines her breasts after menses. Past med hx is unremarkable. Her mother died of ca breast at age of 40 yrs. O/E 1*1 cm rubbery , firm , freely mobile round mass is palpable in UOQ of her rt breast. No ax l nodes r palpable. What is the next best step in mgmt? A) USG B) FNAC C) mammogram D) excisional bx E) observation

FNAC would be the fastest way to find the diagnosis.

ans is mammogram--there is a family hx of ca breast(her mother died of ca breast at age 40) and she is 35, so, baseline mammogram would pick up any other growth , if present . this seems to be a fibroadenoma, so, excisional bx wud be therapeutic and diagnostic too.

Mammaogram is always the first to do in a woman > 20 years of age and presenting with lump in breast. Only time when mammogram is not the first choice is 1) <20 yrs age 2) lactating woman.
