A 39 year male come to your office with his ELIZA report which is showing positive for HIV. On history he told you that he use to take iv drugs but now stop it for last one years. He is living with her girl friend for last 5 year and she is the only sexual partner. He has broken with one before and that is all the sexual contact he had.
On examination you did not find any thing abnormal except rale at the base of right lung.
What will you do next?
1. Call her gf about her positive test
2. Start HAART
3. Screen for T- cell
4. Western blot
5. Viral load
6. Chest Radiograph
7. PPD test

ELISA is screening test of choice, sense of 99.9%, the confirmatory test of CHOISE is W Blot the specificity of 99.9%! No q about this q!

But is your primary concern the HIV status or the determination of the cause of rales?

You know most important thing is to get the dianosis of aids and related complication so first cofirm diagnosis of aids

No,SA it was not an easy Q.In fact this Q may help me avoid making mistakes on the real exam.I would say its a good basic testing knowledge.

Thanks Veer I also try to test the very basic thing but I really not sure that in actual exam what will be the level of difficulty?
Any one had the experice can share that!

I've done a few USMLE CD questions.....questions are basic and don't get into complicated stuff.I'm have no idea whats the level of difficulty in the actual exam though??
