Hi! You might want to check out this site: www.ecfmg.org
If you mean, transcript of records by the medical transcript, then you don't have to worry about it because you don't need to submit a copy. What you need instead is a photocopy of your diploma and send it along with your application form. The ecfmg will directly get a copy of your TOR from your medical school.

when u apply for step 1 ull need to send in 4 things (besides completing the online part of ur application)
1- ur notarised certification statement (form 183)
2- form 344
3-form 345
4-copy of medical diploma with photogragh.
u dont need anything else for step 1/2 .refer to ecfmg.org .they give everything in detail.
