A 16 yr old girl comes to the physician for her first prenatal visit at 12 weeks of gestation. She has not had any immunizations since the age of 5 yrs. She has received the following immunizations at the recommended ages

5- diphtheria- tetanus – pertusis

3- Hepatitis B

1- measles- mumps- rubella

4- Oral poliovirus

Examination shows no other abnormalities. Which of the following is the most appropriate immunization to administer at this visit?

1] DTP
2] DIPHTHERIA [CHILD] - TETANUS TOXOID
3] DIPHTHERIA [ADULT] - TETANUS TOXOID
4] HEMOPHILUS INFLUENZA TYPE B
5] HEPATITIS B
6] INACTIVATED POLIOVIRUS

I am between 2 or 3.

1.DTP - is not given as pertusis is not given for children >5years.
2 & 3 & 6 - Td+Polio is contraindicated in first trimester pregnancy
4.Hib vaccine is not indicated in children >5years

So the answer must be Hep B

  #7

Any age pregnancy is the indication for Hib vaccine.

Hib vaccine is different from Influenza

Hib is given 2,4,6,18 months and not indicated in children > 5 years

Influenza (flu shot) is contraindicated in children< 6 months. It's given for immunocompromised, diabetes, etc but NOT Pregnancy

as she already received the hep b vaccine will she be getting the booster dose now?

  #11

from my knowledge Hep. B protection is usually life long(have not come across booster doses for Hep. B??).

I feel the best answer in this situation is 3.because she didn't receive her booster DT shot.OPV is live attenuated and not given to pregnant mothers.Polio virus can cross the placenta!!

Pertussis is not indicated in this situation.
