A healthy 42 yr old comes to physician for a life insurance evaluation. He smoked 1 ½ pack of cigarettes daily for 20 yrs but quit 10 yrs ago. His father died of MI at 65 yrs age. 93kg [205 lb], 178cm [70 inch], 160/110, 96, 16. Rest of the examination normal. Lab values also normal except serum cholesterol level of 206 mg/dl. What is the greatest risk factor for cerebral infarction in this pt?

1] Genetic profile
2] Smoking history
3] Hypercholesterolemia
4] HTN
5] Obesity

I think the greatest risk for cerebral infarction in this pt is HTN, if HTN would be for a long time, he will develop some kind of arrhytmia(AF)). I don't think his cholesterol level is too high(normal is <200) to cause MI and thus cerebral infurction :?: . May be I'm wrong..... :|

the greatest risk for stroke comes from htn according to UW.

Most occlusive strokes in brain are the result of thrombosis rather than embolism. Hence, the greatest risk for thrombosis would be the greates risk for stroke. Greates risk for thrombosis is HTN according to most path and pathphis books. Therefore, HTN is the the greatest risk for occlusive cerebrovascular dzz or stroke.

hypercholesteraemea does have a chance but we normally

see efffects from this in chronic elvated patients,like older age group

smoked 1 ½ pack of cigarettes daily for 20 yrs but quit 10 yrs agonot a very important risk factor since he quit smoking :wink: . 10 yrs after quiting smoking, the risk is the same as in the general population :idea: .

father died of MI at 65 yrs age
So what grin ? He did not die before 50 :wink: . Not important in this case.

obsese, indeed sad . But ECFMG does not care about that grin . This is not considered a significant risk factor grin .

Doesn't tell me anything sad . A bit high, but not seriously high :wink: . Besides, it doesn't tell me anything about HDL-CT or LDL-CT :cry: . Not a risk factor to considered in this case :wink: .

In conlcusion, the greatest risk factor now is HTN grin . Answer 4.
