62 yr old woman with ovarian cancer comes to ER because of fever for 2 days. 10 days ago she received chemotherapy with paclitaxel and carboplatin. She feels tired and has not had nausea or vomiting. 39.5c [103.1], 100/60, 115. Lungs clear to auscultation. Examination shows a soft nontender abdomen. Hematocrit 32%, WBC 800, plt 105,000. Serum electrolyte levels are normal. Most appropriate next step in management? 1] Plasmapheresis 2] Additional chemotherapy 3] Iv antibiotic therapy 4] Iv corticosteroid therapy 5] Transfusion of 2 units of leucocytes 6] Transfusion of 2 units of packed RBC. My answer is 3.

the pt. seems to have all cell blood levels low. So, you are right, 3 is the best answer, since the WBCs are the lowest.

I wish there was G-CSF or GM-CSF in the choices ...so since they are not there i'll go with IV-AB as well. She is got chemo induced BM suppresion.
