47 yr old nulliparous woman brought to ER by ambulance becos of acute low back pain radiating to posterior leg for 2hrs. the pain began when she bent over at work to retrieve a file from the lowest drawer of a filing cabinet. No smoking or drinking history. On physical examination right paraspinous muscle spasm and pain in the lower back with right SLR at 30 she says that she plans to file a claim for a work related injury. Which of the following is the strongest risk factor for prolonged episode of pain here?

1] arrival for care in ambulance
2] claim that pain is work related
3] gender
4] nulliparity
5] positive SLR test
6] radiation of pain into posterior lower extremity

I chose 5 here.
