I am a PGY-1 resident determined to go into the match to join another program of the same specialty to start all over again. Can I use the ERAS documents such as LOR, Dean's letter, college transcipt that are dated Nov 2003? I understand AAMC number has changed, but i am going to correct it with correction ink by myself.

Also, I am resigining my current position because I got probation. Do program diriectors find this out sooner or later?

Yes, program directors will find this out because according to the ACGME program requirements:

Resident Transfer
1. To determine the appropriate level of education for residents who are
transferring from another reside ncy program, the program director
must receive written verification of previous educational experiences
and a statement regarding the performance evaluation of the
transferring resident prior to their acceptance into the program. A
program director is required to provide verification of residency
education for residents who may leave the program prior to
completion of their education.

As for the ERAS documents, you would have to check with ERAS for that information.

And even if you left a program, you are considered a transfer.
