In my knowledge for modified ECT, the most common complication is memory impairment and for unmodified ECT, the musculoskeletal events comprise the commonest and the most distressing morbidity. These events include compression fractures of the dorsal spine, fractures of long bones, dislocation of shoulder and temporomandibular joints, muscle or ligament tears, and other complications.

Broken bone is complication in the past, but not nowadays 'cause we take some sedaton before ECT If i am not wrong, in some developping countries just a few years ago, they do ECT without sedation.
