You can wear gloves and that should not affect your score .

Dacis, I am very confused about whether or not i should tell sp about the disease names in d/d. Like if there is a sp with increase urination, i am thinking to tell her that she may have Urinary tract infection, or Diabetes or some other problem. for chest pain, i would say it may be because of some problem with your heart, or lungs or reflux disease, or if she gives a h/o panic attacks, then i will include and say "it may be due to a panic disorder or anxiety attacks" Tell me can i use specific terms like urinary tract infection or panic disordes, or GERD, or Parkinsons dis ( if he has it ) or i cant use them and i have to be vague in telling him or her about dx and should tell her just where the problem is, not the disease???. This is a final day please clear this point as soon as possible. Thanks a lot, Sar.

You can use a specific diagnosis if you feel sure about it, but be sure to explain the disease in layman's terms to the patient. If you are not completely sure about the diagnosis (which will happen most of the time), then you can either be vague and let the patient know you want to run some tests first based on your h and p before you give a definitive answer; or you can let the patient know what possibilities you are considering (in this case, you can tell them something like '...I believe you have something going on with your digestive tract..." or "...you maybe experiencing a blockage of blood flow to your brain, but we want to run some tests to be sure of what's going on." After you tell the patient about what you plan to do, ask them if they have any further questions or concerns, and then answer them as you see appropriate. I know the question wasn't directed to me, but I see that you are quite anxious, which is understandable since your exam is tomorrow; so I wanted to give my opinion. Good luck on the exam, don't let your nerves get to you. Once you begin with the first patient, you will start to feel relaxed about the whole thing. Time goes by fairly quickly, so try to be aware of your time management during the patient encounters/note writing. Take care!

Hey Sar, Im glad to see people are actually reading this post. Anyhow, even if you choose to wear gloves, remember to wash your hands. As far as the other question goes, I would straight up tell them as most of the issues they will come in with are common as it is, but make sure to go over it with them. For example, a urinary tract infection is pretty self explanatory. Be careful not to get TOO casual, 'cp b/c of some problem with your heart' is too layman and you may actually risk your dr-pt relationship ifyou put the pts condition too vaguely. Tell them, you could be experiencing your CP for a number of reasons one is angina, which is when blah blah happens, or it could possibly be reflux when your gastric fluid comes into your esophagus where your food normally travels down. Personally, I think if you throw in your medical jargon and explain it, it helps present the issue better, makes you look more competent and hence makes the pt feel a little more confident in you as a doc and feel like theyre understanding is close to yours as far as their condition goes....but you have to explain it well, and make sure to ask if all this making sense. I know all your hard work will pay off, just remember to relax as much as you can. You already know it all, and I know you will do wonderfully. Make sure you are well rested! Best wishes, -2bmd

I agree with 2bmd . You don, t have to tell Pt all the possible Dif. Dg . - but just 2 or 3 . You can use some simple medical terminology - urinary tract infection , panic disorder , diabetes , heart attack etc. . I f you just say that Pt has some heart problems it will sound like you don't know what's going on . Good luck for your exam tomorow .

Yes ravi_online, I got my result in march, and i passed it. I agree with you that this message board is veryhelpful. 2bmd was very helpful to me and she answered all my foolish questions. But i think that helps a lot of exam takers. We discussed it here so that it would be helpful to others also. Whenever someone ask me any question regarding CS exam and prep i refer him or her post. Best of luck to you! Sar.

dev as you know smoking is associated with gastritis, pancreatic cancer, oesophageal cancer, headache, tobacco ambylopia, urinary badder cancer, aging, cerebral atrophy, vascular disorders, mi,atherosclerosis, fatique(co excess),iugr. so if pt. is having any of these symptoms you can council him/her for to quit smoking. ravi
