6o yo female
with widespread diffuse pain with symmetric tender points in both shoulders
Physical exam normal.
Lab test normal..including ESR normal
fatigue, muscle pain positive
most likely diagnosis?

this is a case of fibromyalgia. note the classic symmetric point tenderness in both shoulders.
polimyositis will have evidence of ESR increased.. and on physical exam will reveal proximal muscle weakness.
