A 23-year-old woman drinks several diet colas containing several hundred milligrams of phosphate but no calcium. Which of the following hormonal responses will most likely occur in response to this ingestion ? (A) Decreased 1,25(OH)sD3 and decreased parathyroid hormone (B) Decreased 1.25(OH)2D3 and increased parathyroid hormone (C) Increased 1,25(OH)2D3 alone (D) Increased 1,25(OH)2D3 and decreased parathyroid hormone (E) Increased 1,25(OH)2D3 and increased parathyroid hormone

If you increase the PO4 you decrease the synthesis of 1 alpha hydroxylase in the kidney, so less D3. Also if you increase the PO4 there is less free Ca which would stimulate PTH

The increased PTH will it not induce or enhance the activity of 1 alpha hydroxylase? If so there should be an increase in 1-25-DHCC. So was thinking of answer E.

i'll go with b too ...... as this is just a short term fluctuation ! only under pathological conditioin for eg. primary hyperparathyroidism ,due to excess PTH vit.D level will elevate to significant levels! besides as long as PO4 remain elevated alpha-1 hydroxyalse activity will be suppressed. [dominant in this state]

B. Phosphate will bind available Ca creating pseudohypoCa...PTH will go up to dump extra P and increase transient fall in serum Ca. Drinking soda alone does not provide the necessary Vit D in milk, assuming that the girl does not live in sun shine state :-) Comments on resoning?
