my cousin was involved in a knife fight. he was transprted to ER and arrived with BP 80/60, pulse rate 110 bpm and resp rate 18/m. He was in and out of consciousness on arrival. I noted a chest wound to the left of sternum. what might it be a. cardiac temponade b. ruptured pulmonary artery c. pneumothorax d. perforation of the aorta e. perforation of the left ventricle. I am guessing what the immediate treatment might be.

Depends on the area of the puncture. If it was proximal 1/3 of the sternum could be the aorta, 1/3 medial could be pulmonary artery 1/3 distal could be left ventricle.

Anatomically, RV is behind sternum and extends to the left of sternum. LV is locater further to the left of sternum, Aorta is significantly posterior. The Lung is displaced by the heart to the left of the sternum. So, my cousin likely has had perforation of RV resulting in cardiac temponade. Immediate pericardiocentesis may be livesaving, but surgical repair willneed to be undertaken.

My cousin told me that most common malignant germ cell tumor of anterior mediastinum is cystic teratoma. I know it is not true. But I forgot the answer. It maybe A.choriocarcinoma B.seminoma C.endodermal sinus tumor D.embryonal cell carcinoma
