some programs have later deadlines, so it won't hurt to apply if you have the money and you can have requirements met..LOR from your country will help...I have 2 from my school, and 3 from work here in the US.

US LoR are the best, but if you don't have those, you still need letters so they have to come from somewhere. USCE is great, almost a must, but not necessary for the weaker programs. I am worried, if you quite your job to get USCE, how will you support yourself and your wife? Can your wife work instead?

usmleasr, I do not know what a medical assistant does. If you have direct patient contact, then the anser is yes and no. Stronger programs want you to be an elective student doing a clerkship, the next tier down want at least an observership/externship. Other programs will look on what you are doing (clinical work outside the normal scope of a student) as acceptable.

retroviridae. A medical assistant takes vitas signs, apply injections and vaccinations, takes EKG, spirometries, audimetries, Urinary drugs screens, assists with procedures, wound cleaning, etc. so does it couns? isit still better to do an externship?

An externship shows that you attend medical lectures and that you are in a learning, not just a working environment.
