24 yo FEMALE, c/o left flank pain that radiates to the vagina, comes and goes.
No fever. No other symptoms.

Next step in management?

a. CBC
b. Urine analysis
c. Abdominal X-ray
d. Sonogram
e. Spiral CT Scan
f. MRI
g. IVP

The answer should be the same as the other q: abdominal xRay as the next step in the management since 90% of the stones are of calcium type.

B-hcg. to rule out pregnancy first, bcause if u do X ray on pregnant women.... well, it s not good,

what s next step?

Hi,
Oh, that was tricky, I guess you are right, hCG :shock: should be done first and if no pregnancy is detected then my answer is the Xray.

i think answer is the [B]urine analysis to look for micro haematuria usually associated with nephrolithiasis.....if its positive or clinical suspicion strong enough then spiral ct is done to confirm the diagnosis....but its not the next best step ....spiral ct cud b the answer had the ques asked for the best /most accurate test to diagnose this patient

I m so sorry. Praveen was right.

I should have mention in the question stem that Urine analysis is positive for blood (microhematuria)

then next step?

B-hcg if women.
X-ray if men

if all (-) then next step?

spiral ct scan.

hi, I think in this case initial investigation should be
1 Urine RE
2 HCG to rule out Ectopic

Hcg is the first thing to check . But for ureteric colic due to stones Ct scan is the best .
Please correct me if wrong.
