With your permission, here's another case: 23 yo pregnant female comes to the ER for the first time during her pregancy (33rd week). She complains of severe abdominal pain, fevers and severe dizziness and palor. History: that's her 3rd pregnancy (first at the age of 20 - went normal, 2nd at the age of 22 - the child was underweighted and pale and needed 4 units of blood transfusion within the first 2 days of life). 2 episodes of vaginal bleeding until present that stopped spontaneously. Physical: severe palor, distance from the pubic symphisis to the fundus of the uterus = 20 cm, no fetal heart tones can be heard. Lab: BP = 70 / 40, HR = 200, RR = 25, Rh (-), HB = 6, HT = 24. No father's blood tests available. The most probable dx is: 8) The next most logical investigation is: :wink: The most logical therapeutic measure is: :shock: How would you approach such a case? :lol:
