Early in February, the wife of a 39-year-old male patient calls your office to discuss her husband with you. She is very concerned about him. She states that for the last three and a half weeks, her husband has refused to leave the house for any reason, eat, or shower. He sleeps all of the time and seems to lack interest in anything. She feels that he is depressed and needs help. He does not have a fever or any symptoms of a physical illness. As far as you know, this behavior has never happened with this patient before. He is an accountant and usually an active member of his community. His wife states that he usually has ‘energy to spare’ and is always willing to lend anyone a helping hand. What do you believe about this patient? A. He is probably experiencing a new onset of schizophrenia. B. He is probably experiencing a major depressive episode. C. He is probably experiencing the depressive side of an underlying bipolar disorder. D. He probably has the flu despite the lack of physical symptoms because it is going around. E. He is experiencing a dysthymic disorder.

HE HAS MAJOR DEPRESSIVE DISORDER. HE NEVER HAD ANY SYMPTOMS SUGGESTIVE OF MANIA . HE JUST HAS ENERGY TO SPARE. FOR ONE TO DIAGNOSE SCHIZOPHRENIA, IT MUST AT LEAST BE 6 MONTHS. FOR ONE TO BE DIAGNOSED AS DYSTHYMIA, IT SHOULD BE AT LEAST FOR 2 YEARS WITH PATIENT FUNCTIOINING SUB-OPTIMALLLY. SO, HE HAS DEPRESSION. NOTHING ELSE.
