A 67 yo man with prostatic hypertrophy has a solitary cyst which was identified on renal USG done to determine whethere upper urinary tract obstruction was present. The report indicates that the lesion is a simple cyst. The next step to evaluate this problem shud be..........
a.renal arteriogram
b.renal CT scan
c.IVP with tomography
d.no further studies
e.cyst puncture

my choice is cyst rupture. smiling face

ABSOLUTELY GUYS!!!!!!!!!!!DON'T DO ANYTHING!!!!!!!! grin grin
Most renal masses r simple cysts , the incidence of which is estimated to be atleast 50% in pts. aged 50 yrs or older. US findings consistent with a simple cyst is an adequate evaluation.
The diagnostic w/u shud proceed further only if the cyst contains internal echoes or is of an indeterminate type sonographically.....in which case a CT scan shud be done. If the nature of the mass is still indeterminate after CT scan, Percutaneous puncture of cyst is recommended. Clear yellow fluid that is negative on cytology supports the diagnosis of a simple cyst.
Angiography is no longer used for diagnostic purposes but sometimes useful before the surgery.
IVP is a screening procedure , but it is not used for evaluation of an already defined cyst or mass. Cysts identified by IVP shud be examined furtther by USG.
So, all of u did gooood!!!!!!!!! grin grin
