hi want to share my own answer on my ownQ. I asked my friend, an internist, about the use of estrogen. She said that Premarin or prempro is not being used anymore bec of the increased incidence of breast cancer. If ur pts are still on it then you need to taper the dose first before stopping it.

Hello Tuesday! What's happenning in your world? Besides increasing the incidence of breast cancer estrogen replacement therapy as taking a combination estrogen and PROGESTRONE ..... causes increased incedence of cardiovascular events such as heart attacks, strokes and DVT as well,.... but estrogen alone doesn't increase the risk of getting above mentioned illnesses although it has risk of endometrial hyperplasia.....So Health Institute doesn't recommend to stop the ESTROGEN ALONE therapy but recommended to stop a combination therapy! ....so give estrogen orally and progestrone has to be added to all women who has intact uterus to prevent endometrial cancer!. There are other alternatives for estrogen replacement who can't take estrogen ....like SERMs( Raloxifene, Tamoxifen ect) ..they have protective heart and bone effects but don't have much effect on menopausal psymptoms such as hot flashes, sweats, dyspareunia and irritability as estrogen does. So the best method of treating menopausal symptoms is estrogen replacement therapy!!!....well according to kaplan medical! 8) .....also not to mention estrogens osteoclasts controlling power....which is ....in women estrogen controls osteoclasts' activation...in other words they tell osteoclasts how much bone they have to break down and when to stop ....so estrogen inhibits osteoclast activating factor or otherwise known as IL-1 ( the same thing that causes fever in acute inflamation.....perhaps that's why postmenopausal women gets hot flashes associated with IL-1 and estrogen which alters hypothalamus thermo-regulation) ....for men it's their testosterone that controls octeoclasts activating factor! 8)
