So, i wonder that really makes a difference between a good candidate and a not-so-good candidate. Is it just the way you carry throughout the interview...

It's the way that you answer during an interview. Can you speak English fluently? Programs need to know that to know if you can talk to patients. If you can't answer a program director as to why you want to get a fellowship in GI, then how are you going to tell a patient about their disease? How are you dressed - does your suit fit, did you take it to a tailor to make certain that it fits? Do you need a haircut? Do you smell fresh? Sorry, one guy yesterday forgot this step which doesn't make a good impression? Are you the shrinking violet in the corner not talking to anyone? Are you talking with someone else from your country in your home language, and not talking English to the candidate from another country?
