cilastatin impedes the transformation of imipenem into a nephrotoxic compund in the renal tubules.

It actually inhibits dihydropeptidase in the renal tubules so imipenenen is not inactivated!

Right! that's what i meant, but i couldn't remember the name of that enzyme... Thank you! in conclusion, it has 2 effects: 1. keeps Imipenem active, so that it can be efficient in UTIs 2. protects kidney from being damaged by the toxic inactivated form of Imipenem
