1. If I were asked whether I want to go back to my country, is it better to answer yes or no? What's wrong with saying "At this time I intend to return to my country. However, what I might do in three years is really hard to say. I could get lucky and obtain a waiver position"
2. If I were asked whether I want to pursue fellowship, should I say yes or no? (both at primary care program and traditional categorical program with fellowship opportunities)
I don't know enough about a primary care program. Can you do fellowship following them. The answer should be honest. If you want fellowship, tell them.
3. Is it gonna hurt me if I don't ask too many questions about their program since, to be honest, I don't mind working anywhere in US. In any case it would be better than in my country. Sugar coat this one (lie? :? ) - mention that the material that you found on the web page and in talking with other residents in the program have really answered all your questions. Make certain that the program director believes that you have done your homework about his program! Good luck!

I did answer I wanted to pursue fellowship and the PD went like.. So... you're not really interested in primary care program then :icon_freeze: Oops.
