Does the J1 or H1 visa allow for starting in one specialty (general surgery) and then changing after 3 years to another (plastic surgery)? Which of the two visas has the best possibilities?

J-1 visa - seven years are allowed - sponsored by more institutions.
You can change from an H to a J, but not a J to an H ___________________ bbb - trying to combine common sense and humour into realistic answers, but not going to guess on anyone's chances of getting into a position....
