If you see flat mucosal surface and lymphocytes in the lamina propia what does every good doc think of?

Whipple is what I was thinking of also.

Celiac Disease.

Whipple is cauzed by infection, hence lot's of macrophages instead of lymphocytes which are usually seen in immunological diseases (high sensitivity).

that might be correct. Macs really are part of Whipple. Lymphs would be dominating in Celiac sprue.

agree celiac or tropical sprue.

Yep, celiac or tropical. Then you'd need a history to differentiate between gluten sensitivity and a recent trip to the carribean.

yeah its easy to confuse these 2??
*I was thinking Celiac Sprue with the abnormal D-xylose test.

In my humble opinion, any localized diseases like Sprue here MUST have support from patient's history. No history, no diagnosis. It should be mentioned in differential diagnosis though.
