A 45 yo man presents with a 2 cm firm mass in front of the left ear , which has been present for 4 months.The mass is deep to the skin , and it is painless.The patient has normal function of the facial nerve.Vitals are stable.Wat is the best next step in manangement? 1.Biopsy in the office 2.Biopsy under anesthesia 3.Formal superfical parotidectomy by a surgeon 4.Excisional biopsy 5. Total parotidectomy

why shouldnt the parotid be biopsied under anesthesia?

Biopsy causes scarring, thereby, increase risk of facial nerve and parotid duct injury.

No I think it is not right to take up the patient for surgery unless you have a diagnosis. eg if the mass is benign the surgical extent would be different as oppsed to if the mass is malgnant. You cannot assume that the mass is parotid; it could also be a preauricular lumph node. An FNAC in the office is the first thing to do.
