A 16 y-old girl come to your office with her father complaining of ankle edema, increased abdominal volume, and lack of energy. After examination, you found that she was 6 months pregnant of twins and was hiding it from her parents. Shes also states that she hasn't had any prenatal visits. What will be your decision? a-inform the father b-try to convince her to tell the truth c-take care of her medical problem and ignore the rest d-send them to the social worker e-find the baby's father and talk to him

I will go with c - take care of her medical problems, the rest is irrelevant. She can be considered partially emancipated minor. Age of consent- 16 and older. For certain issues only - prenatal care, STDs, birth control, Substance abuse treatment, you don't have to get guardian's permission.

I am going with "a". Twin pregnancy as far as I know in a primigravida should be considered a risky pregnancy especially when there are signs of preeclampsia. She needs real care and may need to be hospitalized. Someone has to make a medical decision here. Remember that her father who came with her is in the waiting room. "Most states recognize an emancipated minor as a person who meets one of the following criteria: self-supporting and not living at home married pregnant a parent in the military" ETHICS IN MEDICINE University of Washington School of Medicine Even though she could be considered an emancipated child, one can argue that the only criteria she fulfills is being pregnant. And according to Kaplan step1 General principles book p 836 "having children does not make someone emancipated" I will definitely let her know that I am going to talk to her father in her presence about the choices for her care and also ask her if there is anything she doesn't want me to talk about during that meeting.

HAVE CHILDREN WHOM THEY CARE FOR She is not one of these so she is not emancipated next she does not needs any surgical or medical treatments at this moment not (emergency), not a reason to tell her father Parent consent is not required in the tretment of minors in the following instances A>EMERGENCY( YOU CAN NOT FIND THEM AT THIS MOMENT AND DELAY A TREATMENT CAN HARM THE CHILD) (no emergency) B. tretment of STDs( no STDs ) c. Prescription of contraceptives (she is already pregnant) D. MEDICAL CARE DURING PREGNANCY (OK) E. Tretment drug and alcohol depence ( she is not) WE exclude A Father and the baby have no rights at all everything up to pregnant women exlude E send them to the social worker -- she was not raped, abused ,she is a good child just pregnant ) EXCLUDE E YOU CAN TRY TO CONVINCE HER TO TELL THE TRUTH, BUT YOUR JOB is TO TAKE CARE OF HER MEDICAL PROBLEM SO I AM GOING WITH C JUST BECOUSE IT IS MORE IMPORTANT AT THIS MOMENT to TAKE CARE OF HER :oops:

actually ankel edema is due to her pregnancy (i dont see any medical problem in the stem ) if she has no medical problem other than the pregnancy then .. as she is 16 years ( can consent ) SHOULD TELL HER TO INFROM HER PARENTS , (ans b ) telling herr the risks . cant ignore this .

she has no medical problem. uncomplicated pregnacy is a normal event.she has a high risk pregnacy being primigravida and having twins but anyway is not complicated for the moment.the father has the legal right to be informed right away. we're talking about informing not consent cuz there's nothing to consent for the moment.

I agree with Shawn's explanation--pregnancy is not an emergency. She just needs to elevate her legs to avoid the edema. The important things is the the father (family) be told about her pregnancy.

that's an old question it actually tests your knowledge of what partial emancipation is. it is not about medical emergency, it is about a 16 year old in need of prenatal care. for that and also for contraception, std treatment, and substance abuse treatment you don't need to inform parents of 16-18 year old. this was ment to protect confidentiality of a minor with these problems, so they can safely seek treatment and not be afraid that parents will be told. in our case, i would most definitely pick treatment option as opposed to informing parent. don't get me wrong, i am not saying you shouldn't encourage girl to tell the truth, but at the moment (6 months high risk pregnancy, no prenatal care, edema) medical care seems more appropriate. she might be hypertensive and have a seizure right in front of you as you're trying to convince her to tell the truth. the point is, you are not obligated to inform her guardian if she objects. hope this helps. good luck to all!

I think the ignore the rest throws us off...of course we treat this but is even a small amount of edema not normal? The question is asking what should be done morally...like persuade her to tell her father...treating does not fulfill the moral obligations in itself....
