Lay Charles is a black male who become blind at age 7. After his brother drowned. Conversion
reaction is in the differential of his blindness. What else is in the differential diagnosis of blindnes in a 7 year old child beside this and possibly Retinoblastoma although the latter I believe this usually occurs in whites??
Any thoughts?

Remember Lay Charles didnt drown his brother drowned.
But I guess "river Blindness" could be in the differential. I should have said he lives in the U.S.

sth congenital like a deficiency in galactose metabolism --> cataract ?

Mars-Appreciate your thought, but sometimes in this litiginous, meticulous medical field names may be changed to protect the innocent.

Also: In galactosemia and the other rare carbo. disorders:
1)Symptoms occur weeks or months after birth
2)Infant is reluctant to drink milk or milk formula
3)Cataracts develp in only a few months
4)Mental retardation is evident at 6 to 12 months
5)non glucose reducing sugars are found in the urine.

Any other thoughts?? :shock:

Vit A deficiency due to malabsorption ?sth like cystic fibrosis and his brother had also this disease -all this cough, sputum may predispose to drowning-
but...Cf is more likely found in caucasians...

Vitamin A deficiency is a thought ,but might that include dermatitis, dry mucous membranes, and alopecia.
Usually this is seen in developing tropical countries (500,000) kids are aff ected each year. I guess there could be rare instances in this country. I would definitely consider this before CF.
