H/O otitis media few days back ---noncompliance with meds or not treated well.Now pt presents with bulging tymp memb or ruptured tymp membr with pain on pulling pinna ,laterally displaced pinna and a fluctauing mass above or on side of pinna . best initial diagnostc test is; xray mastiod next best step in managemant after xray/ statrt i/v A/B as it can lead to meningitis. most accurate diagnostic test is: tympanocentesis and C/S. Most accurate radiological test is ct or mri to se the extent of involvement (so that surgical debridement and mastoidectomy can b done) Which org involved?SMH (step pneum,morexalla,heam inf) so pencilin is good ,if prev given then add clauvulanate.
