Hi guys, I don't know if this q has been answered before. It's how I remember it from nbme part2: It's about a husband who was on some kind of bussines trip, had a little fun with prostitute and got chlamydia/gc from it. Couple days after the trip he goes to his doc, gets some antibiotic, and everything's fine till his wife gets to the phone and calls the poor doc. She says that she saw her husband leaving doc's office and if he has some kind of std she wants to know about it right now. So what do you do? I've chosen not to tell her unless husband will allow for it. But how does it go with exceptions to confidentiality? Another thing is that when you tell her you clearly interfering with theirs marriage. Any ideas how to act?

dont tell her without pt's permission

have to weigh up breach of patient's confidentiality vs harm to others. Wife could become sterile from contracting gono/chl from him which would be tragic as it is so treatable.

one kaplan q I did considered a nurse with a needlestick injury from a p who was ?HIV-pos. The correct answer was to over-ride the patient's autonomy and order the HIV test (even tho he said no) because of the risk of harm to the nurse if you did not order the test.

first, try to get man to tell his wife!!!!! Best solution. If not an option, can tell wife (protected from being sued) because of harm to her if don't tell.
(exception to normal confidentiality rule)
For more discussion of this see the thread "tickle the brain guys" posted a little while ago in this section.

best wishes
