Symptoms of Carcinoid Syndrome
They are sometimes quite hard to rember, because they seem unrelated: diarrhea, flushing, diseases of the right heart valves and bronchospasm.

So imagine a toilet with this sign on it:

When in DIARRHEA:

Please FLUSH

using the RIGHT VALVE

to avoid BRONCHOSPASM

in the next user.

Cute! -The discrepency was also the most common site. If Im not mistaken we agreed on the small intestine?? Since a lot including myself were reflexivly saying the appendix

Within the digestive system, the most common location of carcinoid tumors is the small intestine near the appendix (ileum). The next most common site is the rectum, followed by the colon (large intestine) and the rest of the small intestine. The 3 account for 90% of all cases. At one time, the appendix was a very common site, but that is not longer true. Only about 7% of all intestinal carcinoid tumors begin in the appendix.

Hey Wannabe!: Please note that the latest edition of First Aid says that the appendix is the most common site for carcinoid tumors.... Can you please quote your reference for the data you just submitted? Thanks.

We had a "big thing" about that and I believe most on this forum will agree its the small intestine.

no i dont agree with you ......
all textbooks and the websites say that

** appendix is the most common site of carcinoid tumour ....

*** terminal ileum is the most common site of carcinoid tumours that can produce carcinoid syndromes

the data is from national cancer docs society. Keep in mind they will not put ileum and appy together in question about carcinoid. As long as you know both, u be good. Also FA is written by students and not really verified with any other source. So it can fall behind the actual facts.

I agree with this last statement. To summarize what can be read or inferred from
- First Aid 2004
- BRS Pathology 1994 and
- Robbin's (latest edition):

1. The appendix is the most frequent site of carcinoid tumor.
2. Carcinoid tumor of the appendix RARELY GIVES METS.
3. Carcinoid SYNDROME is the consequence of the inability of the liver to metabolize serotonin produced by a carcinoid tumor, due, in turn to mets from carcinoid syndrome!
4. Carcinoid tumor of the small intestine, gives mets more frequently than that of the appendix, and therefore THE INCIDENCE OF CARCINOID SYNDROME IS HIGHER IN CARCINOID TUMOR OF THE SMALL INTESTINE.

I understand you discussed this thouroughly, but I would thank you to agree or disagree with each of this items QUOTING YOUR REFERENCES. :wink:

Let's not loose our time discussing on what we've heard in lectures or from others!!

Ah ok, I haven't read that, dr. Wannabe. Thank you, so we've detected a possible mistake in FA.... But Robbin's???? I still suggest that, for the exam, you attach to what is in textbooks.

Dear drstepbystep,
Same reason as above. The websites and journals post the newest research and statistics. Robin's, while the best sourse of general concepts, is sometimes falling behind the current data. Besides, think how many years does it take to publish the book. What ever statistics they have there is probable 2 years old. So take basic concepts, and leave some room for improvement in stats, such as MCC's and percent populatins and M:F ratios.
Just my opinion.
