actually i joined a one day kaplan course in which they give 5 patient simulations....then they grade u for those cases...it was a good experience.the question i have is that i was given a case with palpitations and i suspected thyrtoxicosis, arrythmias , drug abuse, panic disorder. on examination i checked the CVS and Resp system but in my result i was told that i didnt do the exam properly.i want to know as she was a female are we suppose to expose her chest fully while doing examination.
i have my exam on 2nd oct...i would appreciate if anyone can clear my doubts...

For the auscultation of mitral area and for palapation of PMI, ask the patient politely to lift her bra..They would do. For examination of other areas, just aks her to lower the gown. That would be sufficient. Be sure, don't try to slide the edge of the stetho underneath the bra.

In the case of palpitation, i think we need to examine Thyroid and Heart atleast, plus eye for pallor (anemia), auscultation of lungs and palpation of abd.

palpation of abdomen to check any organomegaly in relation heart disease or pheochromocytoma.

As a rule in the US, always examine the heart, lungs, and abdo. The standardized patients won't take off their bras for you, so just make do with asking them to lift up their left breast and make an effort to find the PMI (apex).
