OK, just a quick note. I took Step II CS today, and it really isn't bad. I don't see the need to arrive early to the center, as they don't let you into the place until 8, and then you sit around until 8:15 (the latest you can arrive is 8:30). Everyone is very polite, including the standardized patients and proctors. Lunch was not that great. Don't bring anything besides your lab coat and steth. The cases are all straight forward and relatively easy. I think they are all designed to have multiple possible diagnoses. My only tip would be to watch you time. On several cases I didn't have a chance to "counsel" the patients. Don't know how much that will hurt me. My final advice, put the drape on first, and wash your hands first, just so you won't forget! Hi, thanks for sharing your exeriences! Does that mean the patients do not wear their gowns when you enter? What do they wear then? and did you always do the complete heart and lung examination? If somebody is likely to have an MI, did you ask about sexual history (in this situation it does not seem appropiate to me...) and are their any emergency situations? Thanks, Anja

By drape, I mean a sheet that they have sitting on your stool. It is to cover them up when you examine them. They are all wearing hospital gowns. I tried to do as complete an exam for each system as I could, and I have to admit, I didn't always ask detailed sex histories (or at all). I think you are supposed to, but if it's not relevant, why do it?

thanks for your reply.i dont think a etailed sexual history is always needed,especially if the case is unrelated. i saw a post of yours which said as a rule in the US one should always examine the heart,lungs and abdomen.thanks a lot for that tip.it has solved some of my other worries.but i want to know if it doesnt pertain to the system particularly,you dont have to do a full examination do you? eg in an abdomen case,what do you check in the heart,just pmi and auscultation of mitral area?or do you auscultate all valvular areas?in checking the lungs in this patient,do you just auscultate the lung areas directly? thanks in advance.

I tried to do as complete an exam as I could. Remember, most patients are without findings (although some do have them), so you can just go through the motions of auscultating the heart or looking for the PMI.Same with liver span. I mean, in reality, we don't check all these things all the time, but it's an exam. Just becareful to not run out of time like I did and not be able to counsel them.

retroviridae: What kind of positive findings you were faced with and what we should expect? Would you consider PE more important then Councilling? Thanks
